Manic-depressive disorder

Manic-depressive disorder, also known as cyclic psychosis, or bipolar disorder for short, is an affective disorder (afleetive disorder), also known as mood disorder (Mood disorder), is one of the most common psychiatric diseases. Mania and depression happen to be the opposite of two different groups of emotional disorders. The main clinical feature is high or low emotion, accompanied by corresponding cognitive and behavioral changes, which does not lead to personality defects. There is a clear interval between episodes, when the mental state can be completely normal. The disease is characterized by recurrent episodes, self-remission, and no residual symptoms, and a few patients can be prolonged and become a chronic process.

The prevalence of bipolar disorder in China is lower than that in western countries, and this difference may be related to the different national characteristics, social and cultural background and diagnostic criteria.

Bipolar disorder is a different disease in traditional medicine. Manic type is equivalent to "irritability" and "mania" in traditional Chinese medicine, while depressive type belongs to "depression" and "epilepsy". The depression type belongs to the category of "depression evidence" and "epilepsy evidence".

Manic and depressive disorders of the identification and treatment

(a) Specialized disease-specific formula

1. Longmai Anshen Tang is suitable for postpartum mania. Sheng long teeth 30g (first decoction), Sheng long bone 30g (first decoction), magnetite 30g (first decoction), cinnabar lg (punch), Poria 20g, Huaibai wheat 30g, baked licorice 12g, jujube 7, white peony 30g, danshen 15g. Bitter ginseng This product is the dried root of the leguminous plant bitter ginseng, containing a variety of alkaloids, the treatment of manic psychosis, the bitter ginseng dosage began to be 9 to 12g per day, 2 to 3 times after meals, gradually increase the dosage. The dosage of bitter ginseng is 9~12g per day, and the dosage is taken after meal 2~3 times, and the dosage is increased gradually. 40 cases were treated, and the total efficiency was 82.5%. Research on the identification and treatment of manic-depressive disorder

Luo Zhengxin divided mania into 3 types:

Phlegm-fire internal disturbance type: equivalent to acute mania; solid-heat internal junction (internal fire blazing) type: equivalent to mild mania; fire-inflamed and injured yin type: equivalent to chronic mania.

Depression is categorized into 2 types:

Liver-qi stagnation type: equivalent to mild depression, and when the symptoms are severe, it is equivalent to acute depression; long-term depression and injury to the yin (qi and yin injuries) type: equivalent to chronic depression.

Ding Dezheng classified manic-depressive psychosis depressive state into:

Heart qi deficiency type: treated with Yiqi nourishing heart and tranquillizing the spirit soup (Astragalus, Codonopsis, Atractylodes macrocephala, Poria cloud, jujube seed, northern schizandra, roasted licorice, Yuanzhi);

Liver qi deficiency type: treated with tonifying qi and nourishing the liver soup (Astragalus, Codonopsis, Longtooth, Paeonia lactiflora, dogwood meat, Angelica sinensis, Atractylodes macrocephala, Poria, jujube seed, roasted licorice, Chaihu);

Liver-Qi Depression type: treated with Liver-Sparing and Depressing Tang (Angelica sinensis, Chaihu, Paeonia lactiflora, Yujin, Radix et Rhizoma Glycyrrhizae, Pores, Pores, Zao Ren, Radix et Rhizoma Glycyrrhizae, Succinium amanitum);

Kidney Qi deficiency type: use Kidney Nourishing, Essence Enriching, and Le Shen Tang (Radix Rehmanniae Praeparata, Astragalus, Cornu Cervi Pantotrichum, Chinese yam, Lycium Chinense, Zao Zao Ren, Cyperus Rotundus, Poria, Dragon's Tooth, and Yuan Zhi).

Wei Shanchu believes that the main link of this disease is the stagnation of liver qi, phlegm and stasis, and its treatment should be to dredge the liver and dissolve the stagnation, remove phlegm and eliminate stasis. Due to the chilling or heating of the qi stagnation to the point of alternating or intermingling of cold and heat, the treatment must be tailored to the evidence and the treatment should be carried out in a peaceful manner. The treatment of manic type is to regulate qi, soften the liver, dissolve depression, and clear phlegm and blood stasis, while the treatment of melancholic type is to dredge the liver, dissolve depression, and warm phlegm and blood stasis.
