Information required for on-site certification of medical practitioners

Materials required for on-site assessment of clinical assistant physicians:

(1) Original and photocopy of my invalid identity certificate. Self-invalidation identification includes self-identity card, military officer's card, civilian cadre or soldier's card, travel card and identity card (candidates from Taiwan, Hong Kong and Macao) and passport (foreign candidates).

Candidates' registration photos should be 2-inch bareheaded half-length black-and-white photos in the past 6 months (this photo is used for preparing for the "Doctor Qualification Certificate" and "Doctor Practice Certificate" at the same time and cannot be changed).
