Feng pointed out in "Classic Prescription of China Decoction and Medicine": "The reason why the sick human body has this general regular reaction of six meridians and eight classes is not mainly because of external diseases, but because of the internal function of the human body's disease-resistant mechanism." At the same time, it is pointed out: "The main spirit of TCM syndrome differentiation and treatment is to emphasize the general treatment of diseases on the basis of the general regular response of the sick human body. It is "an etiological therapy that adapts to the human body's disease-resistant mechanism" and attaches importance to the internal role of the sick body in understanding the disease; In the treatment of diseases, we should pay attention to the disease-resistant function of the diseased body, that is, the ability of self-rehabilitation. Feng's clinical practice always embodies the concept of "governing the people" by classical prescriptions.
Ear diseases are more common in shaoyang disease.
For ear diseases, the medical classics school based on Huangdi Neijing thinks that its acute symptoms are mostly related to hepatobiliary diseases, and Bupleurum is often used to treat Shaoyang disease. The classic school based on Treatise on Febrile Diseases takes the Eight Classics and Six Classics as cognitive tools, and holds that ear diseases mostly belong to semi-exterior and semi-interior syndrome, and the excess syndrome is mostly shaoyang disease. Hu Xishu's Treatise on Febrile Diseases and Miscellaneous Diseases points out in the explanation of Article 263: "Shaoyang disease is a syndrome of yang half exterior and half interior, and yang heat is between the chest and abdomen, and here it is half exterior and half interior, so it can't enter the interior, nor can it leave the exterior, so it can only go up between the orifices. "Bitter mouth", "dry throat" and "dizziness" mentioned in Article 263 of Treatise on Febrile Diseases, and "deafness" and "red eyes" mentioned in Article 264 belong to the diseases of the orifices. The treatment of otopathy, whether it is nerve, virus, inner ear or outer ear, starts from shaoyang disease in half exterior and half interior, and the sum of Bupleurum chinense is used for treatment, which is in line with the natural ability of human body to treat diseases. This is the way of treating diseases by classical prescriptions.
Recognition of Xiaochaihu Decoction
Traditionally, Xiaochaihu decoction is the main prescription to treat shaoyang meridian and fu-organ evil, and it is the representative prescription to embody "harmony". Clinically, it is widely used in various diseases such as exogenous internal injuries, and is widely used in various diseases such as fever, digestive system diseases, mental and emotional diseases, respiratory diseases, gynecological diseases, etc. Feng believes that it seems to be clinical to understand, interpret and use Xiaochaihu Decoction in this way. However, from the perspective of corresponding prescriptions and syndromes, it is difficult to master and the curative effect is uncertain. Feng advocated eight basic principles to interpret the six meridians, and the prescription was based on syndrome differentiation and treatment, which was simple and effective.
All lesions have disease sites that respond to the disease. According to the disease sites, exterior syndrome, interior syndrome or semi-exterior syndrome can be distinguished. All pathological changes have a dispute between good and evil. According to the ups and downs of justice in this struggle, we can distinguish between yang syndrome and yin syndrome. According to the combination of disease location and yin and yang, six meridians can be distinguished: Taiyang, Yangming, Shaoyang, Shaoyin, Taiyin and Jueyin. According to cold, heat, deficiency, excess and corresponding symptoms, prescriptions and syndromes can be further distinguished. Xiaochaihu decoction is suitable for Xiaochaihu decoction syndrome, which belongs to shaoyang disease syndrome, and shaoyang disease should be distinguished first in clinic. Shaoyang disease is a syndrome of half exterior and half interior. In order to identify it, Feng put forward two main points in his book Zhang Zhongjing's Medical Interpretation: First, "Heat stagnation in the exterior and interior, which can neither reach the exterior nor enter the interior, will inevitably force the mind, so bitter mouth, dry throat and dizziness are natural reactions. Therefore, any disease with bitter mouth, dry throat and dizziness can be judged as shaoyang disease. The second is, "So the syndrome differentiation of shaoyang disease is more correct than before, so it is better to ask for the side." In other words, it is necessary to supplement the exclusion method, because the exterior is easy to know and the yin and yang are easy to judge. Any evidence except exterior and interior is, of course, half exterior and half interior yang syndrome, that is, Shaoyang disease. Regarding Xiaochaihu decoction, Feng also pointed out the key points of syndrome differentiation: "exterior-interior heat syndrome, or bitter mouth and dry throat, dizziness, chest and hypochondriac pain, not thinking about diet." 」
Bitter mouth, dry throat, earache and deafness in this case are obviously caused by heat stagnation in half exterior and half interior. Combined with pulse count, Xiaochaihu decoction is undoubtedly regarded as the treatment of Shaoyang disease. At the same time, the patient also has obvious dry mouth and sore throat, which is considered to have Yangming internal heat, so it is further divided into Xiaochaihu plus gypsum, orange stem soup combined with shaoyang and Yangming disease. The usage of 24 grams of Bupleurum root comes from "half a catty of Bupleurum root is used in the prescription, divided into three parts, and each part is equivalent to eight yuan" (Hu Xishu's Treatise on Febrile Diseases). The next day, Coicis Semen and Patrinia scabiosaefolia were added to enhance the power of clearing Yangming. The last prescription is asarum, which is intended to "promote blood circulation and remove blood stasis" and clear the orifices. The prescriptions and syndromes are consistent, but they are fully effective.
