What good method treats scapulohumeral periarthritis?

The treatment of scapulohumeral periarthritis has two main purposes: relieving pain and restoring joint mobility.

1.? Relieve angular pain in pain stage and maintain joint range of motion.

2.? Recovery of joint range of motion in stiff stage

3.? In remission, the range of motion of the joint should be restored. If the joint is stiff, it needs surgical release.

Measures:

1.? Relieve pain

(1) oral drugs? Such as non-steroidal anti-inflammatory drugs and analgesics (NSAIDs), have limited efficacy.

(2)? Partially closed? Cortisone is commonly used, and it is reported that the long-term effect is not ideal.

(3) ? Local anesthesia? It has been reported that injecting local anesthetic around the suprascapular nerve or in the intermuscular groove of brachial plexus can relieve the pain symptoms of scapulohumeral periarthritis, but the curative effect lasts for a short time.

(4) Joint expansion method? Also known as hydroforming, 40~50ml of liquid (mixed with local anesthetics such as bupivacaine and lidocaine, and corticosteroids) is injected into the joint to reduce the volume of the joint cavity. Joint expansion has a good effect on relieving pain and restoring joint mobility.

2.? Relieve joint stiffness

① ? Manual release under anesthesia is to release the adhesion tissue around the joint by manual release under anesthesia to restore the mobility of the shoulder joint. However, manual release is more difficult, and different techniques may have different effects. In addition, manual release has complications such as fracture, joint dislocation, rotator cuff injury, brachial plexus injury and soft tissue injury around joints. Therefore, it is necessary to be cautious to adopt this treatment method.

② ? Surgical release, including open surgery and arthroscopic minimally invasive surgery. With the progress of arthroscopic minimally invasive surgery technology and equipment in recent years, arthroscopic release has gradually become the main means to treat scapulohumeral periarthritis, and achieved good clinical results. Arthroscopic release of shoulder joint mainly includes excision of inflammatory synovium in rotator cuff space, release of upper ligament of shoulder joint, coracoid ligament, anterior joint capsule, release of subscapular tendon and separation of joint capsule under shoulder joint, which has obvious curative effect on relieving scapulohumeral periarthritis pain and restoring joint mobility after operation.
