Teacher evaluation.

(1) Application Form for Examination of Traditional Medicine Teachers (attachment1);

(2) The original and photocopy of my ID card;

(3) Three two-inch bareheaded frontal half-length photos;

(4) Proof of academic qualifications or academic ability;

(5) A copy of the instructor's doctor's qualification certificate, doctor's practice certificate and professional and technical post qualification certificate, or a certificate issued by the county health and wellness committee that he has been engaged in clinical work of traditional Chinese medicine for more than 0/5 years;

(6) The original and photocopy of the notarized teacher-student relationship contract of traditional medicine;

(7) Learn the original notes from the teacher.
