The scope of surgery for vulvar cancer depends on the clinical stage, the lesion site, the degree of differentiation and infiltration of tumor cells and the patient's physical condition.

The scope of routine surgery for vulvar cancer should include radical vulvar surgery and bilateral inguinal lymph node dissection.

1. Generally, one-stage operation is adopted, that is, radical vulvar surgery is performed immediately after inguinal lymph nodes are cleared;

2. If inguinal lymph nodes are positive, pelvic lymph node dissection should be added;

3. If the focus is small and biased to one side, radical excision of vulva and inguinal lymph node dissection are feasible; However, for vulvar cancer with clitoris, even if the lesion is small, it should be operated according to the conventional scope.

The precancerous lesion of vulvar cancer is called vulvar intraepithelial neoplasia, including vulvar atypical hyperplasia and carcinoma in situ. Treatment depends on the patient's condition and age, such as freezing, laser and surgical resection of local lesions. No matter which method is adopted, long-term follow-up is needed.
