1. Collect basic information:

5. Occupation and social participation assessment:

6. Make a rehabilitation plan:

7. Implement rehabilitation intervention:
After the rehabilitation evaluation, the evaluator will carry out rehabilitation intervention according to the formulated rehabilitation plan. This may include physical therapy, occupational therapy, speech therapy, psychological support, social support and other rehabilitation interventions aimed at helping patients recover their functions and improve their quality of life.
8. Regularly evaluate and adjust the plan:
Rehabilitation evaluation is a dynamic process, and the evaluator needs to evaluate the patient's rehabilitation progress regularly and adjust the rehabilitation plan according to the actual situation. This helps to ensure the effectiveness and pertinence of rehabilitation intervention.
