He needs to bring the notarial certificate, the apprentice's ID card, the household registration book, the original academic certificate, and the copies of all the materials that the teacher needs to provide when handling the notarial certificate.

3, pay attention to blank and copy

When filling in the traditional medical master-apprentice relationship contract, if the notarization date can be determined, you can print the signing date, notarization date and master-apprentice teaching time.
