How to write the record of cholecystectomy?

After successful general anesthesia, take the supine position, routinely disinfect the operating field and spread sterile towels. Incision was made about 6cm below the costal margin of the right upper abdomen, which was cut layer by layer, electrocoagulation was used to stop bleeding, protective towel was put on and penetrated into the abdomen. The size of gallbladder is about 10*5*3cm, and there is no adhesion of transverse colon and omentum around it. The gallbladder wall is thick and the liver is normal. Anatomy of the gallbladder triangle, separated to the confluence of the three tubes, fully separated the anatomical relationship of the three tubes, ligated the cystic duct about 0.5cm away from the common bile duct, removed the gallbladder, and ligated the cystic duct and the gallbladder artery respectively. Thoroughly stop bleeding, check that there is no active bleeding, and two placenta compress the gallbladder bed. Check the gauze instrument, close the abdominal cavity, and sew the incision layer by layer. After cholecystectomy, more than ten stones were found, the largest being about 1* 1.5CM, 1* 1CM.

The family examined the gallbladder and sent it to pathology. The anesthesia was satisfactory and the operation was smooth. The patient returned to the ward after the operation.
