1. Surgical treatment

After partial complete resection, new tumors still appear in the original surgical site or other parts of the breast or even the contralateral breast. This is not a real primary tumor recurrence, but a second primary tumor. It is unfounded to say that excision of breast fibroadenoma will lead to another tumor.
(1) Timing of surgery For unmarried women, if the diagnosis is basically clear, under close follow-up, selective surgical resection before marriage or after marriage can be considered according to the wishes of patients; For patients who want a baby after marriage, pre-pregnancy surgery is recommended to help avoid pregnancy and breastfeeding, because pregnancy and breastfeeding will accelerate tumor growth; If the tumor is found after pregnancy, it should be surgically removed within 4~6 months of pregnancy. No pregnancy, breastfeeding, trauma, etc. That can promote the growth of the tumor, which suddenly grows faster in a short time, so we should operate in time. It is best to avoid premenstrual period and menstrual period during operation.
(2) Surgical methods The traditional surgical resection is to select a surgical skin incision for complete surgical resection according to its aesthetics and convenience. The arc-shaped incision along the edge of areola heals, and the scar is small and not so obvious visually. The incision on the lower margin of breast can be considered as multiple. The principle of layered incision should be carried out during operation. The skin and subcutaneous layer can follow the direction of dermatoglyphics, and the breast layer should be cut radially around the nipple to reduce the damage of mammary duct. The whole tumor should be completely removed by surgery. The disadvantage of traditional surgery is that it will leave scars on the skin incision and affect the beauty of the breast. For patients with large tumor resection area, which affects the cosmetic effect of breast, breast plastic surgery and reconstruction should be combined. Minimally invasive surgical resection generally chooses breast fibroadenoma with definite diagnosis. It is to poke a hole (about 3mm) in a hidden place such as armpit or areola. Under the guidance of ultrasound or molybdenum target, the tumor was removed by Mammoton or Echo vacuum-assisted rotary cutting system. The pain is small, leaving only a hole mark of about 3mm after operation, and the recovery is fast. The incision doesn't need sewing, so there is no need to take out stitches. Multiple tumors can be removed simultaneously through one incision, especially tiny tumors that can't be touched clinically. Disadvantages are high cost, local bleeding and subcutaneous ecchymosis, and sometimes complete resection cannot be guaranteed.
Because of the possibility of clinical misdiagnosis and missed diagnosis, surgical specimens should be routinely examined pathologically. Appropriate treatment should be given according to the results of pathological examination. For traditional surgical specimens, intraoperative frozen sections can also be used for pathological examination.
After fibroadenoma surgery, there is still a similar probability of fibroadenoma regrowth in other parts of the breast, so we should still pay attention to regular physical examination and imaging examination after surgery.
