How does TCM treat hypertension based on syndrome differentiation?
(1) hyperactivity of liver yang. It is characterized by dizziness, headache, head swelling, red eyes, bitter taste, irritability, sometimes insomnia, red tongue, thin yellow fur and thin and smooth pulse. The treatment should be calming the liver and suppressing yang. Prescriptions: Tianma Gouteng Decoction and Longdan Xiegan Decoction. This type is more common in early hypertension.
(2) Yin deficiency and yang hyperactivity. It is characterized by dizziness, headache, heaviness, tinnitus, forgetfulness, fever of five senses, palpitation and insomnia, red tongue, thin and white fur and rapid pulse. It is advisable to nourish yin and reduce yang. Prescription: Qi Ju Di Huang Tang or Bai Zhi Huang Tang. This type is equivalent to the second compensatory period of hypertension.
(3) Deficiency of both yin and yang. It is characterized by dizziness, tinnitus, palpitation, backache and leg weakness, unsteady gait, dry mouth and throat, chills and cold limbs, insomnia and dreaminess, nocturnal polyuria, impotence and nocturnal emission, muscle cramps, pale or red tongue and thready pulse. Treatment should nourish yin and yang. Selected prescription Jingui Shenqi Pill. This type is equivalent to the third decompensation period of hypertension.
(4) Wind-phlegm obstructing collaterals. It is characterized by numbness of hands and feet, anxiety, skewed mouth and eyes, unfavorable language, even hemiplegia, white and greasy tongue coating and slippery pulse. It is advisable to extinguish wind and resolve phlegm. Prescription: Ditan Decoction or Banxia Baizhu Tianma Decoction. This type is equivalent to hypertension complicated with cerebral thrombosis.
(5) Qi and blood rise and fall. It is characterized by sudden fainting, unconsciousness, clenched teeth, firm hands, red face, shortness of breath, excessive phlegm, irritability, body heat, Huang Gan or greasy fur and strong pulse. The treatment should be calming the liver and calming the wind, nourishing yin and suppressing yang. Prescription: Jiawei Zhengan Xifeng Decoction. This type is equivalent to hypertension complicated with cerebral hemorrhage.
