New traditional needling method

Fengchi point comes from Lingshu fever, alias hot compress, which belongs to the gallbladder meridian of foot shaoyang, and is the meeting of hands and feet shaoyang, yang maintenance and yang Qiao Mai. It is one of the most commonly used acupoints in clinic. According to the literature, Fengchi point can be used to treat many diseases, especially those caused by pathogenic wind. It is also widely used in clinic, such as treating cold, headache, cervical spondylosis, trigeminal neuralgia, hypertension, stroke, neurasthenia, epilepsy, optic atrophy, myopia, tinnitus, heel pain and so on. There are different opinions on the acupuncture angle and depth of Fengchi point. According to most ancient records, it is appropriate to directly stab Fengchi point at 3-7 points. Acupuncture Zi Sheng Jing and Acupuncture Dacheng proposed acupuncture points 1 inch 2, and acupuncture along the meridian proposed 1 inch 5.

Professor Yan Junbai, a famous Chinese medicine practitioner in Shanghai, believes that acupuncture or moxibustion at Fengchi point (indirect moxibustion) can stimulate human yang, dredge meridians and regulate qi and blood. As the saying goes, "treating wind first cures blood, activating blood circulation and extinguishing itself", so the yang is strong, the blood is smooth, and the cold evil is naturally easy to dissipate. Therefore, the clinical application of Fengchi point is by no means limited to the above diseases. As long as patients have symptoms of yang deficiency and insufficient love between good and evil, they can start from Fengchi point.

Professor Yan often uses the following seven kinds of acupuncture methods to acupuncture Fengchi point.

Traditional shallow needling method

1. The tip of the needle inclines to the tip of the nose, which is mainly used to treat many diseases. Puncture 0.5- 1.0 inch at the tip of nose with a needle of 1.5 inch. Induction should have local pain or spread to the top of the head, temporal region, forehead and orbit, and can be used to treat headache, cervical vertigo, ear vertigo, common cold, unexplained fever, allergic rhinitis, sinusitis, tinnitus, facial paralysis, apoplexy sequelae, epilepsy, cervical lymphadenopathy, thyroid dysfunction and other diseases. The nose is the opening of the lungs. Acupuncture at the tip of nose can enhance its effects of expelling wind and cold, dispersing lung qi and relieving exterior syndrome, and inducing resuscitation. Treating exogenous wind-cold exterior syndrome is more effective.

Special deep needling method

2. The needle tip inclines inward and goes deep into the opposite eye socket, which is mainly used for head and face diseases. Take a 2.5-inch filiform needle, twist it slightly, and stab it deeply near the cervical vertebra in the opposite direction of the eye socket, which can reach more than 2 inches, and the place where the needle tip reaches is equivalent to 1 inch under the eye socket. At this time, there is a strong sense of acupuncture, which goes up along the gallbladder, reaches the ear temporal part and the top of the head, and then goes forward to the forehead and reaches the eyes. After getting angry, patients can feel headache, dizziness, heavy head, dizziness, facial pain, earplugs and other symptoms relieved or eliminated.

3. Punch the tip of the needle directly on the same side of the alar to treat the sequelae of stroke. The acupuncture method is the same as 2, and the position of the needle tip is equivalent to

2.3 inches under the eye socket, the needle feeling is the same as 2, or it is conducted upward, lower limbs and trunk. It has good curative effect on hemiplegia or numbness of limbs after stroke.

4. Acupuncture in the direction of throat is suitable for pseudobulbar palsy. Take a 2-inch filiform needle and pierce it toward the throat 1.5-2.

Inches. The deep anatomy of Fengchi point shows that blood vessels and nerves are abundant, and acupuncture can improve the blood supply of vertebral artery. In addition, Fengchi point is the main point of gallbladder meridian, and "gallbladder governs decision", and deep needling in the direction of throat can enhance its efficacy of refreshing the brain, opening up the mind, clearing the throat and benefiting the throat.

5. The acupuncture direction is slightly outward (temporal side), which is mainly used for migraine and severe insomnia. Take a 3-inch filiform needle, the acupuncture direction is opposite to the throat and slightly outward, and the needle is inserted 2.5 inches, and the needle sensation radiates to Bai Yang point on the same side of the head. Fengchi point obliquely pricks outward, reaching the affected area directly, which has the function of promoting blood circulation, expelling wind and relieving pain. It not only has obvious curative effect on migraine caused by exogenous wind-cold, but also has excellent curative effect on headache caused by hyperactivity of liver yang, blood stasis and phlegm, and deficiency of qi and blood.

6. The needle tip penetrates below the ipsilateral angle to treat diseases such as trunk, neck, limbs, throat and trachea. The depth of acupuncture can reach 2-3 inches under the ipsilateral corner of the mouth, and it passes through the cervical vertebra, and the place where the needle tip reaches is equivalent to 3.3 inches under the eye socket. At this time, the acupuncture sensation can reach the ipsilateral upper limb, lower limb and trunk downwards, and can locally spread to the vicinity of the throat.

Penetrating needling

7. Acupuncture into the contralateral wind pool (Fengfu) indicates symptoms such as cervical spondylosis. Take a 2.5-inch filigree needle and insert it into the opposite wind pool (Fengfu) 1.5-2 inch. Acupuncture at the contralateral Fengchi point (Fengfu) is similar to Jiaji point of cervical vertebra, which can dredge meridians, harmonize qi and blood, and improve blood supply to the brain, so it is more effective for cervical spondylosis of vertebral artery type.

Professor Yan's experience: acupuncturists should carefully feel the blood circulation under the needle and the patient's reaction. If there is no needle sensation after deep needling, he can withdraw the needle and change the direction slightly before needling. With the gradual deepening of acupuncture, there may be 1 needling sensation (shallow needling) and two needling sensations (deep needling), which are stronger than the former. Deep needling is effective for intractable diseases. But when the needle goes in, it is still necessary to communicate with the patient from time to time. If the needle feels too strong, or the patient has dizziness, nausea, cold sweat, palpitation and other discomfort symptoms, it is necessary to stop the needle in time.

Safe depth of Fengchi point

Medical accidents caused by improper acupuncture at Fengchi point are not as many as those at Fengfu point and Dumen point, but there are also reports. Acupuncture too deep, posture deviation can be complicated with subarachnoid hemorrhage. The middle and deep part of Fengchi point is the life center-medulla oblongata. Improper acupuncture can lead to serious consequences. Therefore, human anatomists have also done a lot of research on the safe depth of Fengchi point.

Yang Zhanlin's study on the depth of direct needling at Fengfu, Dumen and Fengchi points shows that there is no danger of entering the skull when the needle is directed to the opposite eyeball 1-2 inches; (2) When the needle goes straight to the same depth as the needle to the tip of the nose, there is no danger of entering the skull, and the left and right penetration (horizontal penetration) is deep (50-75 mm); ③ The direction of needle insertion is the easiest to enter the cranial cavity to the tragus or its front edge, corresponding to the direction of vertebral artery entering the cranial cavity from the foramen magnum. The depth from the contralateral pleura to the cranial cavity is as follows: (465,438+0.265,438+0.3.82) mm for males and (43.88 4.665,438+0) mm for females; Right wind pool (to the left tragus): the average value of male is (4 1.2 1.3.6) mm, and the average value of female is (43. 1.94.75438+0) mm, so the safety depth should be within this range. However, the data are for reference only, and the clinical application needs specific analysis of specific problems. For example, considering individual differences, thin people should appropriately reduce the depth of needle insertion, and inexperienced people can use moxibustion massage. Over time, a certain therapeutic effect can be achieved.

Professor Yan summed up in practice: the different needle insertion direction and depth at Fengchi point will lead to different needle feelings and different effects. In other words, different needle insertion methods should be adopted to achieve different therapeutic effects. If we want to verify whether it meets the treatment requirements, we need to judge it through the objective reflection of "qi reaching the diseased position"

Theoretical research and application of scalp acupuncture

Editor: "Fang's scalp acupuncture" is a new acupuncture therapy summarized by Fang in medical practice. Because the prescription doctor first proposed it when he was working in Lantian County, "scalp acupuncture" was called "Lantian scalp acupuncture". 1976, scalp acupuncture was mainly used in the scalp of the head, so it was renamed scalp acupuncture. In order to distinguish it from other provinces and cities in the same period, Fang renamed it "Shaanxi scalp acupuncture" Fang Peng Yun, the founder of scalp acupuncture in Shaanxi, originally studied in the nine-year medical school run by Germans in China. He had a solid foundation in western medicine and was called "xi 'an Dao" at the beginning of liberation. Later, in 1958, due to a special case, I paid attention to the study of acupuncture points with special functions on the head. From 65438 to 0969, scalp acupuncture (then called scalp acupuncture) was widely used, studied and popularized when it was decentralized to rural areas. 1976, Fangzheng named his scalp acupuncture treatment system "scalp acupuncture", thinking that the part of the scalp covering the hair is the main part of scalp acupuncture. In the study of scalp acupuncture, Fang's theory of luring viscera and images is an in-depth development of TCM viscera syndrome differentiation and image theory. Perfecting the theory of total meridian and final meridian is a supplement and development to the traditional meridian theory. Because of its unique curative effect, Fang's scalp acupuncture theory won the third prize of national scientific and technological progress.

Fang introduced:

Kun, male, born in Huaiyang, Henan, was born in 1909. 1936 graduated from medical college of Henan university and engaged in western medicine surgery in his early years. 1948 began to teach himself Chinese acupuncture. 1952, I studied in the acupuncture experimental class of the Central Ministry of Health. 1June, 958, I published a paper entitled "Try Acupuncture to Replace Painkillers and Cardiotonics" in Xi 'an Health Newsletter, and edited Acupuncture Calendar, Acupuncture Calendar, Meridian Flow and Soul successively. The "acupuncture calendar" is adapted into BASICI program and input into microcomputer. Fang devoted himself to the treatment of diseases with the combination of traditional Chinese and western medicine, conducted in-depth research on new acupuncture methods such as scalp acupuncture, and accumulated rich clinical experience, all of which were published in monographs.

I. Academic characteristics and medical expertise

In clinical practice, Fang found that there are acupoints in the head that are not found in meridian theory and nerve theory. Through the treatment and observation of tens of thousands of patients and 150 diseases, scalp acupoints have been continuously improved. * * * Seven acupoints were found, and 2 1 cerebral cortex function which can be used to treat diseases was located in the projection area of scalp surface, and its theory was discussed.

1. Acupoint location and indications

(1) latent image: Its acupoint area is like a miniature of a human body lying on a coronal seam, a sagittal seam and a herringbone seam.

① Head and neck: It is about 3 cm long and 2cm wide before the coronal point, in which the head is 2 cm long and the neck is 0cm long and 0 cm wide 1cm.

② Upper limb: the coronal sagittal point to the pterygoid suture is 1 1cm, in which the coronal sagittal point is 2 cm from the shoulder, 3.5cm from the shoulder to the elbow, 3.5cm from the elbow to the wrist and 2 cm from the wrist to the fingertip. (The left and right upper limbs are the same).

③ Trunk: 140 1X 1 from the coronal sagittal point to the cusp of herringbone suture, which is divided into three parts: back, waist and buttocks. The back is 6cm long and 3cm wide (divided into upper, middle and lower parts, each accounting for 2cm). The waist length is 4 cm, the width is 2 cm (upper and lower waist are 2cm each), the hip length is 4cm, the width is 3cm, and the upper and lower hips are 2cm each.

④ Lower limbs: from the point below the herringbone seam to the star point, * * * is 9 cm long and is divided into buttocks, knees and ankles. From the point below the herringbone seam to the buttocks, it is 1.5cm, from the buttocks to the knees, 3 cm from the knees to the ankles and 1.5cm from the ankles to the toes.

(2) Concealed viscera: it is a miniature of the left and right internal organs and skin of the human body lying across the front hairline. Its head faces the frontal midline and its foot faces the frontal angle, and it is divided into upper, middle and lower triple energizers, with a total length of 6.5cm. The upper energizer includes the chest, viscera, upper limbs, skin sensation and brain thinking on the diaphragm, accounting for 3 cm. 2 cm in front of the upper focus is the head and neck, and its width is above the hairline 1cm and below 0.5cm. 2cm from the forehead midline, 2 cm above the forehead, 1cm from the forehead midline and 3.5cm above the forehead. Two points are connected and divided into lower arm, upper arm and hand, each accounting for about 0.500 1. After focusing, it is 100 1, 2 cm above the hairline, and 0.5 cm below the hairline is the chest. ② Moderate lesions: visceral sensation above umbilicus and below diaphragm, with a length of 65,438 0.5cm, a width of 65,438 0.5 cm above hairline and a width of 0.5 cm below hairline. ③ Lower coke: visceral skin sensation under umbilicus, reproductive system, * * * 2cm long. The lower abdomen, buttocks and buttocks are in front of the lower focus 1.5cm, in the international focus 1.5cm, and in the international focus 0.5cm. 0.5cm after the lower focus, the International Subosseous 1 cm is from the knee to the ankle, and the International Subosseous 1 cm is the foot (the left and right acupoints are the same).

Indications: visceral and skin paresthesia, especially pain, touch, cold, heat, numbness, itching and tightness. Such as stomach spasm, cholecystitis, diarrhea, dysmenorrhea, colic, irregular menstruation, trigeminal neuralgia, spontaneous sweating, palpitation, autonomic nervous disorder, endocrine disorder, dermatosis, urticaria, neurodermatitis, rosacea, psoriasis, eczema, allergic rhinitis, etc.

(3) Visceration and inverted image: located in the projection area of sensory center and motor center of cerebral cortex on the scalp surface, that is, the functional positioning area of anterior sulcus gyrus and posterior sulcus gyrus on the scalp surface. It is mainly used to treat diseases with abnormal sensory and motor center functions.

(4)2 1 acupoint. The function of cerebral cortex is located in the projection area of scalp surface. ① Speech projection area of motor language center, indications: motor aphasia, dysphonia, stuttering, tongue muscle paralysis, pseudobulbar palsy, lip muscle paralysis, brain retardation, tongue tremor, etc. (2) thinking-located between the left and right frontal angles. Note: Except for "thinking", the above acupoints are bilateral symmetry.

2. Acupoint selection and acupoint matching method

(1) Corresponding Acupoint Selection: When a certain part of the body is sick, six acupoints are selected from the corresponding parts of the lower limb for hiding the elephant, hiding the viscera and hiding the inverse, and if the leg is sick, the corresponding parts of the lower limb are selected.

(2) Imitating the body for acupoint selection: According to the theory of acupoint selection based on the differentiation of meridians, viscera images and yin-yang and five elements, imitating various methods of acupoint selection by body acupuncture, and carefully applying them in "hiding images", "hiding viscera images" and "anti-viscera images" for highly concentrated treatment. For example, if you have epigastric pain and vomiting, you can focus on the stomach in the "fu zang" or the corresponding part of the "zhongwan" point in the latent image.

(3) Specific acupoint selection: that is, selecting acupoints according to the main functions of central acupoints. Such as tinnitus and deafness, take auditory points.

(4) Mi-style acupoint selection: that is, selecting acupoints symmetrically, overlapping or crossing the left and right limbs in the latent image and viscera.

(5) Acupoint matching method: ① Six acupoints are matched at the corresponding positions of "latent image" and "latent viscera". (2) The inverted image is matched with the corresponding parts of the dirty. ③ Matching the corresponding parts of the latent image and the inverted image. (4) The dirty and non-dirty parts should be matched. ⑤ The latent image and viscera are matched with the central point. ⑥ Scalp acupoints should be combined with other acupuncture methods.

Second, the choice of medical records

Case 1: Fistula syndrome (sequela of spina bifida)

Zhang XX, female, 32 years old, from Mengcun Commune, Lantian County, Shaanxi Province. 1972 September 12 First time to see a doctor.

1972 On a certain day in February, the patient had fever, cough, general pain and severe pain in limbs. After three months, I felt cold in my lower limbs, and then I was paralyzed, bedridden, my right leg was heavy, my food intake was reduced, and I was thin. In August of the same year, I went to a hospital in Xi 'an and was diagnosed as: ① sequelae of myelitis. ② Spinal arachnoiditis. Usage: Dibazole and vitamins (dosage unknown). After taking the medicine, his condition did not change and he was told to go home for treatment. Come to our hospital for treatment today. The patient was thin, his lower limbs were cold and weak, and he could not stand. He needs two people to help him.

Prescription: both lower limbs (double) are hidden, and the lower half (double) is inverted.

Course of treatment: use flying needle method and keep the needle for 45 minutes? 1 hour The condition improved day by day, and the patient could come to the hospital alone for the eighth time, and was basically cured by acupuncture for 9 times. 1973 April 14 follow-up, able to participate in normal labor.

According to the sequela of myelitis, it belongs to the category of flaccidity syndrome in traditional Chinese medicine. The clinical manifestations are weakness of patella, thin muscles, numbness of skin and hands and feet. This case is characterized by temperature-sensitive pathogen, burning yin and body fluid, irregular acupoints of Yangming meridian and exhaustion, resulting in paralysis. The "latent image" of scalp acupuncture is the epitome of human body lying on coronal seam, sagittal seam and herringbone seam, and it is the general movement center. Clinical practice has proved that it has good curative effect on motor system and vascular system diseases. The experimental observation in the Department of Physiology of Shandong University shows that it can improve the excitability of motor cells in the anterior horn of spinal cord. Therefore, the treatment of the disease adopts the latent image and the corresponding parts, and cooperates with the inverted image to achieve good results.

Example 2: Tremor (encephalitis sequela)

Pan X, female, 4 years old, from Pancun, experimental commune, Luyi County, Henan Province. First visit on September 3rd, 1972.

The child suffered from pneumonia at the end of June this year and was hospitalized in the local hospital 1 several months. After discharge, there are sequelae: limb twitching, whole body trembling, tongue sticking out, staring, unable to speak, eating and swallowing difficulties. He was treated with Chinese and western medicine, but the effect was not obvious.

Prescription: scalp acupuncture, covering the head and covering the dirty focus head, and talking about the main points.

Course of treatment: fly needle to stab directly, and leave the needle for 30 minutes. After the first diagnosis, the child will cry; The second diagnosis, adding the inverted elephant mouth point, the child's crying became soft; In the third diagnosis, acupuncture points of upper and lower limbs were added. After nine treatments, four days' rest and returning to the clinic, the child kept running and playing, eating food, twitching limbs, sticking out his tongue, glaring and other symptoms disappeared, but his speech was unclear. After five times of acupuncture, the child would be called "father" and "mother" and basically recovered.

According to "Fu Xiang" as an effective acupoint for treating sports diseases, "Fu Zang" has a good curative effect on diseases of sensory system, and the disease is located in the brain, so the corresponding head is taken. Speaking point, the projection area of cortical language center on the scalp surface, is mainly used for motor aphasia. The above points can promote the recovery of encephalitis sequela and achieve satisfactory results.

Example 3: Chest obstruction (coronary heart disease)

Chen XX, female, 38 years old. 198 1 year165438+1first visit on October 9th.

1980 In the summer, the patient began to have chest pain and chest tightness, which gradually worsened, especially fatigue, accompanied by shortness of breath, general weakness, emaciation, poor breathing, dull complexion, dark purple tongue, thin white fur and heavy pulse. Examination: ECG circumference prompt:

Myocardial strain; ② Poor coronary blood supply. Blood lipid: cholesterol 200mg%%, triglyceride 1 13 mg%. Cerebral rheogram: the vascular tension of each cerebral artery is increased and the elasticity is poor. Fundus: arteriosclerosis stage Ⅱ. Blood pressure is 90/60mmHg.

Prescription: scalp acupuncture therapy. The heart and chest of the viscera, the corresponding parts of the focus in the viscera, and the corresponding parts of the image and the inverted image are called six grades.

Course of treatment: once a day, every time 10 is a course of treatment, and the needle is kept for 30 minutes each time, with two days' rest for five times, and * * * is treated for four courses. Results of reexamination: ECG: generally normal. Blood lipid: normal. Left ventricular function: generally normal, significantly improved compared with 8 1 year101October 30th. Blood pressure 120/80mmHg. There was no significant change in rheoencephalogram, chest radiograph and fundus. The patient's chest pain disappeared consciously and his chest tightness improved. Only when I hug him, I feel short of breath, and my spirit is good. Eating and defecating are normal.

According to the syndrome of chest obstruction, it is caused by stagnation of qi and blood, obstruction of meridians, and loss of heart support. Western medicine coronary heart disease department. Treatment should promote blood circulation and dredge collaterals. The experiment shows that the acupoints corresponding to scalp viscera burying and viscera dumping have the functions of reducing blood fat, reducing sympathetic nerve excitability, reducing invasive heart rate, reducing myocardial oxygen consumption and increasing stroke output to increase coronary perfusion, thus improving myocardial ischemia, that is, promoting blood circulation to remove blood stasis, dredging arthralgia and relieving pain, so the disease can be cured. 40 patients with coronary heart disease were treated with this method from 65438 to 0982. The total effective rate of symptom disappearance and remission was 96.438 0%. The effective rate of ECG improvement was 87.5%, and blood lipid decreased significantly (P
