How to observe contralateral renal function after radical nephrectomy

Radical nephrectomy is the first choice for the treatment of localized renal cell carcinoma, and it is also the gold standard for treatment. Generally, it is required to free the kidney outside the perirenal fascia, and remove the fat capsule, kidney, lymphatic tissue and upper ureter in one piece. Radical nephrectomy is harmful to patients, and contralateral renal function should be observed after operation.

After radical nephrectomy, routine indwelling catheter can be used to monitor 24 patients.

H urine volume, to judge the healthy side renal function. First of all, check whether the catheter is unobstructed, which may be renal insufficiency in the healthy side or reactive temporary renal insufficiency caused by surgical stimulation. Observe the time, urine volume and color of the first urination after operation. If a large amount of hematuria is discharged during the first urination, notify the doctor immediately. Then observe the urine volume, color and nature every day, and take samples for testing if necessary.

In order to prevent urinary tract infection from affecting the recovery of renal function, the urethral orifice should be disinfected twice a day and the drainage bag should be replaced in time. The position of the drainage bag is generally below the pubic symphysis.

Tips: the main indicators reflecting renal function

The examination indexes reflecting renal function mainly include endogenous creatinine clearance rate, blood urea nitrogen, blood creatinine, hemoglobin and red blood cell count, urine specific gravity, urine osmotic pressure, urine phenol red excretion test, etc. Among them, creatinine clearance rate, blood urea nitrogen and serum creatinine mainly reflect the filtration function of glomerulus and are the most important indexes to judge renal function.
