Pneumonia is lung inflammation caused by different pathogens or other factors. The main manifestations are fever, cough, shortness of breath, dyspnea and lung moist rales. It can happen all year round, especially in winter and summer. It can occur at any age, mostly in infants under 3 years old, and it is the first cause of infant death in China.
This disease belongs to the category of "pneumonia and cough" in traditional Chinese medicine.
Etiology and pathogenesis
External causes include wind-cold and wind-heat. Because children are not full of qi, their lungs are delicate and turbid.
Exogenous pathogens attack the lungs, causing lung qi stagnation, body fluid congealing into phlegm, phlegm blocking lung collaterals, and airway obstruction, leading to dereliction of duty, lung collaterals failing to declare, while water and fluid have no right to transport, stagnate in lung collaterals, congealing into phlegm, and airway obstruction, so cough, shortness of breath, and phlegm in throat may occur; Excessive pathogenic heat, body fluid becoming phlegm, phlegm blocking the airway, phlegm rising with qi, there are excessive heat, polydipsia, cough, excessive phlegm, and even rumbling in phlegm. Pathogenic qi is abundant but vital qi is insufficient, and Jue Yin's wind is shocking; Deficiency of heart yang and rapid pulse are critical symptoms. In the late stage of the disease, the positive overcomes the evil, but it soon recovers. The love of the positive deficiency and evil produces the syndrome of lung heat deficiency or disharmony between camp and health.
The disease is located in the lung and often involves the spleen, but it can also escape from the heart and liver. Its pathological mechanism is lung qi stagnation, and phlegm heat is the main pathological product.

Syndrome of wind-cold blocking lung
Syndrome: aversion to cold and fever, no sweat and no thirst, cough and asthma, thin and white phlegm. Red tongue, thin white fur and tight pulse.
Treatment: Warming the heart and activating the lungs, relieving cough and resolving phlegm.
Prescription: Xiaoqinglong decoction plus gypsum soup. If the nose is blocked, add Flos Magnoliae and Fructus Xanthii to spread the lung and dredge the orifices; For those with excessive phlegm, Xu Changqing, Raphanus sativus and orange are added to relieve phlegm and cough.
Syndrome of wind-heat blocking lung
Syndrome: Fever, aversion to wind, slight sweating, cough, shortness of breath, excessive phlegm, thick or yellow phlegm. Thirst, red throat, red tongue, thin white or yellow fur, and floating pulse.
Treatment: clearing heat and dispersing lung, resolving phlegm and relieving asthma.
Prescription: Maxingshi dry soup. Adding Bupleuri Radix, Scutellariae Radix, Fructus Gardeniae and Radix Isatidis for clearing away heat and toxic materials; Fritillaria thunbergii, Fructus Trichosanthis, and concretio silicea bambusae are added for relieving cough and resolving phlegm for patients with excessive cough and phlegm; Trichosanthes kirilowii and rhubarb are added for constipation, so as to clear away heat.
Syndrome of phlegm-heat blocking lung
Symptoms: excessive heat and irritability, phlegm in the pharynx, thick yellow phlegm, shortness of breath, wheezing in the nose, or cyanosis in the mouth and lips. Red tongue, yellow greasy fur and slippery pulse.
Treatment: clearing heat and dispersing lung, resolving phlegm and relieving asthma.
Prescription: Wuhu Decoction and Granule Jujube Xie Fei Decoction. Wuling decoction consists of dried soup of Maxingshi and refined tea. Maxing Shigan decoction has the effect of dispersing lung qi and relieving asthma, while fine tea has the effect of clearing heart fire and resolving phlegm. Together with Lepidium and Jujube, it can clear lung-heat and reduce qi, and enhance the effects of clearing away heat and resolving phlegm, clearing lung-heat and relieving asthma. Adding Scutellariae Radix, Fructus Forsythiae and Fructus Gardeniae for clearing away heat and toxic materials; Adding Bulbus Fritillariae Cirrhosae, Bambusa Bambusae, and Cortex Mori for clearing away heat and resolving phlegm; For patients with severe asthma and constipation, increase the number of yellow and morning glory seeds, clear the fu-organs and purge the lungs; Radix Salviae Miltiorrhizae, Flos Carthami and Radix Paeoniae Rubra are added for those with blue face and lips.
Yin deficiency and lung heat syndrome
Syndrome: long course of disease, low fever, hyperhidrosis, flushing, dry cough without phlegm. The tongue is red and dry, with thin coating and thin pulse.
Treatment: nourishing yin and clearing lung.
Prescription: Shashen Maidong Decoction. For patients with severe lung heat due to yin deficiency, Artemisia annua and Cortex Lycii are added to nourish yin and reduce fever; Scutellaria baicalensis Georgi and Anemarrhena asphodeloides are added for clearing lung heat; For patients with severe cough, Bulbus Fritillariae Cirrhosae and Folium Eriobotryae are added to moisten lung and relieve cough; Radix Rehmanniae, Radix Angelicae Sinensis and Fructus Cannabis are added to nourish blood and moisten intestines for those with dry stool; For those with obvious night sweats, keel and oyster are added to stop sweating.
Deficiency of lung and spleen qi syndrome
Symptoms: long course of disease, low fever, shortness of breath, excessive sweating, weak cough, loss of appetite, thin stool, sallow complexion, listlessness, no temperature in limbs, pale tongue, thin and white fur, and thin and weak pulse.
Treatment: Yiqi Jianpi.
Prescription: Ginseng and Schisandra Decoction. For hyperhidrosis, add Astragalus to replenish qi, consolidate exterior and stop sweating; Adding Rhizoma Pinelliae, Pericarpium Citri Tangerinae and Arisaema cum bile to eliminate dampness and phlegm; Radix Asteris and Flos Farfarae are added for severe cough to moisten lung and relieve cough.
2. Change syndrome
Syndrome of heart-yang deficiency
Symptoms: Sudden pallor and cyanosis, purple lips, shallow breathing, tepid forehead sweat, cold limbs, asthenia and vexation, blood stasis under the right hypochondrium, thin and white tongue coating, purplish texture, weak and rapid pulse.
Treatment: Warming and nourishing the heart and yang, saving the adverse and strengthening the body.
Prescription: Shenfu Long Mu Jiuni Decoction. Beware of yang deficiency, accompanied by pale face, blue lips and tongue, and obvious blood stasis under the right rib, angelica, safflower and salvia miltiorrhiza can be added to promote blood circulation and remove blood stasis and dredge blood vessels; If both qi and yin are deficient, Shengmai Powder should be added to nourish yin and suppress yang to save adverse reactions.
Syncope yin deficiency syndrome
Symptoms: high fever, delirium, fidgety, limb twitching, strong mouth, eye lifting, crimson tongue, blue handprints, reaching the key point, or shooting nails to puncture points.
Treatment: calming the liver, calming the wind, clearing the heart and opening the mind.
Prescription: Jiao Ling Gouteng Decoction combined with Niuhuang Qingxin Pill. Jiao Ling Gouteng Decoction has the effects of cooling liver, calming endogenous wind and promoting fluid retention, while Niuhuang Qingxin Pill has the effects of clearing away heat and toxic materials, awakening the mind and inducing resuscitation, and the combination of the two has the effects of clearing away heat and toxic materials, calming endogenous wind and inducing resuscitation.
