The application for a health certificate requires a fasting blood test, usually in the morning.

Health certificate application process

①Fill in a health certificate collection notice at the registration office

②Pay at the cashier in the lobby (Window No. 1)

③Photography registration at the registration office (Room 107)

④Medical examination (Room 103)

⑤Fluoroscopy in the radiology diagnostic room (opposite to Room 103)

⑥Collect fasting blood in the blood collection room (Room 101)

⑦Take a stool sample and put it in the stool sample box (next to Door 101)

Please keep it for yourself after the physical examination Collect the notification form and invoice with a good health certificate.

Go to window No. 2 in the lobby at 2:30 pm the next day after the physical examination to collect the notification form and invoice with the health certificate
