What is the current situation and prospect of TCM development?

Three authoritative figures of Chinese medicine talk about the prospect of Chinese medicine!

Deng Tietao (tenured professor of Guangzhou University of Traditional Chinese Medicine, doctoral supervisor, executive director of the All-China Society of Traditional Chinese Medicine, a famous old Chinese medicine practitioner in China):

In the 20th century, some people in China's cultural and academic circles didn't have a comprehensive evaluation and understanding of traditional culture, and thought that in order to develop Chinese medicine, it was necessary to use the model and theory of western medicine to help Chinese medicine. Under the influence of this view, Chinese medicine, whether in medicine, teaching or research, draws lessons from the model of western medicine, showing a strong tendency, that is, bringing Chinese medicine into the evaluation system and track of western medicine. However, because Chinese and western medicine are two different academic systems, the model of western medicine has brought more constraints than help to Chinese medicine. The history of more than half a century has proved this point, and it is time to wake up today.

Western medicine is micro-medicine, while Chinese medicine is macro-medicine, both of which are sciences. But now everyone only recognizes that the western scientific model is scientific. Although Chinese medicine can cure diseases that cannot be cured in the west, it is "unscientific" not to conform to this model. I wonder if this is cultural western hegemonism.

All Chinese medicine practitioners should review, study and explore the classics and cultural essence of Chinese medicine, which is the correct direction of modernization of Chinese medicine. Don't make a mistake, mistakenly think that transforming traditional Chinese medicine into western medicine is the modernization of traditional Chinese medicine. Don't think that bringing Chinese medicine into the American market is modernization and internationalization. Wrong, we didn't develop Chinese medicine to make it acceptable to the west. This not only involves national self-esteem, but also makes it clear that modernization is not the goal, and serving the people is the goal. First of all, we should consider the population of 65.438+0.3 billion. Protecting the health of China people is our contribution to the world.

Swim forward (Doctor of Medicine, University of Melbourne, Australia, Doctor of Traditional Chinese Medicine, Chengdu University of Traditional Chinese Medicine):

As far as I know, foreign medical circles generally believe that medicine can be divided into mainstream medicine and alternative medicine. In China, western medicine has now achieved the status of mainstream medicine. Mainstream medicine recognizes the existence of alternative medicine because human life is limited, which is a realistic need to alleviate illness and prolong life. Western medicine can't conquer the world at least for now. In this case, it is necessary to find other methods in the existing experience of human struggle against disease.

Many ethnic groups have their own traditional medicine. Traditional Chinese medicine combines medical technology with China's philosophical thought, expresses symptoms in philosophical language, and guides the treatment process with theory. It is the most systematic and oldest method in alternative medicine, and it can be said to be the oldest and most vital medicine. The successful treatment of SARS last year is a very convincing example.

Deng Tietao: SARS is a kind of epidemic febrile disease. Chinese medicine has a long history of treating epidemic febrile disease and has accumulated many successful experiences. Last year, two affiliated hospitals of Guangzhou University of Traditional Chinese Medicine mainly treated SARS with Chinese medicine, and the effect was remarkable. None of the 36 SARS patients admitted to the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine died, and none of the medical staff was infected. The vast majority of patients have no sequelae after discharge. The average antipyretic time of patients was 2.97 days, and the average hospitalization days were 8.86 days (excluding those who were discharged automatically).

These cases were diagnosed as "SARS" by western medicine, and were confirmed to be cured by western medicine after treatment by Chinese medicine, and all of them had strict medical records.

In our country, the phenomenon of attaching importance to the west and neglecting the middle is serious. Some achievements have been made in western medicine propaganda, while some achievements have been made in Chinese medicine propaganda. Let's say "I don't know if it's reliable." Even if it is confirmed, we should try to deny it. In fact, hemorrhagic fever is more serious than SARS. Epidemic hemorrhagic fever (EHF) is a national key project in the Seventh Five-Year Plan, which is treated by the Zhou Zhongying Research Group of Nanjing University of Chinese Medicine. The mortality rate of Chinese medicine group was 65,438+0.65, 438+0.65, 438+0%, and that of western medicine group was 5.08%.

Liu Lihong (Doctor of Medicine, Nanjing College of Traditional Chinese Medicine, Professor of Basic Medical College, Guangxi College of Traditional Chinese Medicine):

It is doubtful that Chinese medicine has fallen to such a state. I remember when I just graduated, I was engaged in clinical practice in a Chinese hospital. This Chinese medicine hospital has a clear stipulation that patients with fever should be treated with Chinese medicine. If the fever doesn't go down within three days, they must take western medicine. Why does the Chinese medicine hospital not stipulate to use western medicine to reduce fever for 3 days, but must use Chinese medicine? Is the real Chinese medicine what we see now?

10 in recent years 10, one of the most frequently asked questions in the field of traditional Chinese medicine is why the clinical efficacy cannot be improved. The theory of TCM has been formed for more than 2000 years. Will it be unable to provide effective guidance for clinic because of backwardness? So someone put forward the problem that the theory of traditional Chinese medicine lags behind the clinic. What I want to ask is, do you really understand the theory of traditional Chinese medicine? If the reason is wrongly attributed to theoretical backwardness, then we will form a real retrogression and backwardness. The problem lies not in theory, but in our understanding.

In western medicine, there is a huge technical intermediary between theory and application, and the whole modern science including physics, chemistry and biology is serving this intermediary, which makes the application of medical theory very convenient. Chinese medicine does not have this intermediary, and it depends entirely on the doctor himself. In the face of modern science, elites can be copied. Chinese medicine does not have this convenience, and the theory is no matter how good. If you can't grasp it, it's still zero. The backwardness of Chinese medicine is the backwardness in this link. The present situation of TCM lies in our own understanding and application skills of theory. This theory is not only applicable to the past, but also can solve the problems of 2 1 century. SARS is an example.

In Neijing, doctors are divided into on-the-job and full-time doctors. Zhang Zhongjing said, "If you go to work, you will not be treated, but you will be treated." At present, most people's understanding of Chinese medicine is from the level of illness; The modern Chinese medicine education we adopt is just a way to train people to work.

Swim forward: Indeed, due to the progress of natural science, various advanced technologies have been applied quickly in western medicine, so the diagnostic technology of western medicine has made rapid progress, but the progress of its treatment is much slower than that of diagnosis. A big reason is the slow discovery of drugs. Drug inventions in western medicine usually find that the body lacks or exceeds a certain substance, and then chemically synthesize an approximation to make up for it. Do experiments on animals first, and then try them on people. After problems occur, either improve, reduce side effects or stop taking drugs.

From the beginning, Chinese medicine has been experimenting on the basis of human body, which has been based on experiments for thousands of years. The development history of western medicine is not long, especially the history of drug treatment is only more than 200 years. In this sense, we say that Chinese medicine is safe and effective. It is not that Chinese medicine has no side effects, but that Chinese medicine has long recognized the toxicity of drugs, and through the compatibility and processing of different drugs, the harm of toxicity to human body has been reduced. In foreign countries, it often happens that drugs blindly used by Chinese medicine practitioners in Fang Yaozhong are toxic substances, because Western medicine has no concept of compatibility.

Deng Tietao: The problem now is that the relevant departments in China are also using western medicine to manage Chinese medicine. Good drugs are unqualified in the drug inspection department, such as Niuhuang Angong Pill, which made Phoenix TV reporter Liu Hairuo wake up after three months in a coma, but now it can't be approved for production because it contains heavy metals.

Foreign experiments from animals to people have gone through a long time, but we have to do the opposite. This has been a clinical practice in people for thousands of years, but now it doesn't work. We have to go back and let the mouse nod.

Many people engaged in drug trials in China in the early days were experienced old Chinese medicine practitioners. When they are old and gradually withdraw from the historical stage, most of the new generation of drug inspectors have received western medicine education. Without the theory and experience of Chinese medicine to manage Chinese medicine, I am afraid that less and less Chinese medicine will be allowed to be used in the future.

Why do people have no confidence in Chinese medicine? Because many people think that Chinese medicine must be reformed in order to modernize. What is needed now is the combination of clinical efficacy and new technology to realize the modernization of traditional Chinese medicine. We need a lot of "hardcore Chinese medicine practitioners", that is, doctors with profound theoretical foundation of Chinese medicine and excellent clinical practice who can treat difficult diseases. Behind a prosperous scene, Chinese medicine lurks a crisis of decline!

Liu Lihong: How many students who can go to Peking University and Tsinghua apply for the College of Traditional Chinese Medicine? I don't think so. "It's good that you don't waste it, even if it is fine and subtle, it will be passed on to shallow people."

The so-called modern Chinese medicine education is actually an education that imitates western medicine. The subjects are getting more and more detailed, and many students have been trained, but what about the teaching quality? Over the years, senior students have asked me to give a lecture on how to learn Chinese medicine well before my internship. Why? After studying for 4 years, I still don't know what to use to deal with the internship.

When we advocate scientific research and modern Chinese medicine education, we are completely filtering Chinese medicine through the sieve of modern science, and the essence of Chinese medicine has been filtered out. At present, the problems in traditional Chinese medicine are caused by the problems in educational inheritance. In the modern education mode, there are only instrumental teachers, not masters. If you want to learn Chinese medicine well, you must have a biography of the teacher. This science does not have the permeability of modern science, especially in the application of technology. It is not realized through technical intermediary, but must rely on people to study hard. Therefore, in the process of education, we really need to set an example.

Regarding the experiment, in Huangdi Neijing, we didn't see Huangdi asking Qi Bo, how did you find your theory of Yin and Yang? Did you find it through a mouse experiment? Traditional Chinese medicine does not use things other than the human body to do exterior syndrome experiments in the modern sense, but there are subtle and profound internal syndrome experiments in traditional culture. It is the combination of internal syndrome experiment and rational thinking that constructs the theory of traditional Chinese medicine. Can you think of things like meridians and acupoints just by thinking?

Now there is a terrible strange phenomenon in the field of Chinese medicine, that is, the classic education of Chinese medicine is gradually weakening. Most Chinese medicine schools have changed the classics into elective courses. One of the foundations of supporting classic elective courses is to conduct public opinion surveys. Many people think that classics are of little significance, and they are all marked with "X". You think the classics are nothing, which just shows that you have learned nothing in the classics. Only the modernization of traditional Chinese medicine is allowed, not the traditionalization. I think this Chinese medicine will be finished soon.

Swim forward: Different people may have different attitudes towards the modernization of traditional Chinese medicine.

In order to make it easier to manufacture and sell, pharmaceutical companies hope to develop more tablets and other drugs that are convenient to take, and tend to simplify the formula; Research institutions may hope to find out the pharmacology of Chinese medicine as soon as possible and explain the curative effect of Chinese medicine with the scientific standards of western medicine; Moreover, many Chinese medicine hospitals are willing to deploy more modern scientific and technological instruments and instruments, hoping that they can cure any diseases and perform any operations.

These three aspects are all part of TCM modernization, but they are not the core part of TCM modernization. The modernization of traditional Chinese medicine should meet people's modern needs while retaining the essence of traditional Chinese medicine (dialectical treatment). Improving the repeatability, measurability and convenience of treatment should also be the goal of TCM modernization.

Deng Tietao: The way out for TCM hospitals lies in whether there are high-level TCM talents. There is an urgent need to train real Chinese medicine talents, and there is no way out. How can turning Chinese medicine hospitals all over the country into second-rate western medicine hospitals help China's health in the world?

Before liberation, there were 400,000 to 500,000 Chinese medicine practitioners in China, but now the number has not increased, while the number of western medicine practitioners has increased from less than 30,000 to more than 5 million. In terms of universities, how many western medicine universities are there? How many Chinese medicine practitioners are there? Take Guangdong as an example. There is only one university of traditional Chinese medicine, and the others are all universities of western medicine. There used to be a real mentoring class, but now this relationship is not recognized. There is no such mentoring education system at all, and the most precious inheritance of Chinese medicine is lost.

The medical expenses in the United States reached 1035 1 billion dollars in 1996, accounting for more than 14% of GDP. Can China's population scale withstand the western medical model? To solve the problem that everyone in China enjoys medical care, I think it is necessary to vigorously develop the cause of Chinese medicine.
