(1) Prepare relevant documents and materials:

1, apprentice (apprentice)

(1) ID card; (2) household registration book; (3) the highest academic certificate; (4) the highest degree certificate; (5) The Master-apprentice Relationship Contract of Traditional Medicine (in triplicate) can be printed if the notarization date can be determined. 2. Lecturer (Master)

(1) ID card; (2) household registration book; (3) the highest academic certificate; (4) the highest degree certificate; (5) Qualification certificate of medical practitioners; (6) Practice certificate of medical practitioners; (7) technical title certificate; (8) Proof of clinical and teaching work.

Note: (5) and (6) are different, one is the qualification certificate and the other is the practice certificate. If the teacher's technical title does not reach the deputy chief physician or above, the clinical and teaching work permit shall specify 10 years and above working years. All the above materials must be original, and the notary office requires copying on the spot. Those who bring their own copies don't count. However, since you need a copy to go to the Health Bureau for filing after notarization (and the Health Bureau only needs a copy, not the original), since the teachers have taken out the original, you should take the opportunity to make more copies. However, the notary office is embarrassed, because the copies they ask for are free, so you don't want to print one more. Try to copy that yourself. )

(2) Notarization process:

1. Bring all the documents required above. The most important thing is that two people must be present at the same time. In the process of handling, if one or two unimportant documents, such as household registration book, are missing, you can discuss with the notary and make them up, but people must come.

2. Copy the relevant documents in the copy room of our department free of charge.

3. On-site handling is to sign various documents according to the requirements of notaries.

You can get the notarial certificate one week after all the formalities are completed.
